OSQA is the Open Source Q&A System. It is free software licensed under the GPL, and you can download the source code for OSQA from a Subversion server. OSQA is originally based on CNProg, an excellent Chinese Q&A web application written by Mike Chen and Sailing Cai. OSQA is written in Python and powered by the Django application framework.

For those who want an enterprise-quality solution, you should definitely look at AnswerHub is designed and built here at DZone by the same team that maintains OSQA, but the two systems share no code in common. OSQA was our first-generation effort, and AnswerHub came later and radically improves scalability, security and theming.
