He discusses failure modes for Scrum Master, Team, Product Owner and Management and also looks at failure modes in sprint-planning, daily Scrum, and sprint-review. He also examines how to approach these failure modes.

Scrum is not at all hard to implement. We have been doing successfully for past 1 year. The effort involves in bringing all stakeholders to agree to follow this process.
The mindset needs to change and organization should see visible benefit.
