When is it OK to refactor or should it be left the hell alone?

Know a well-named method from a bad one? Seen code that makes The Daily WTF readers blush? What about one that's more subtle? Weigh in on the argument surrounding one, simple, boolean method. When is it OK to refactor this or should it be left the hell alone?
